| Google Forms | Free | can be created from any google account, results input into a spreadsheet There is an add-on feature available that can merge individual results into individual PDFs, but the set up does take a 'lil elbow grease. With the AutoCrat addon, the forms can be set up with a template for a "mail merge" that will generate a PDF of the submissions that can be stored in Google Drive and emailed to one or more persons. The down side of AutoCrat is that it can be cumbersome to set up the merge template. The merge template will require adjustments any time a new question is added or changes are made to the form. Google does not allow for any payment integration. |

Signing up for Google Account Using Existing Email
Having access to Google Forms and their Spreadsheets of form submissions requires having a Google Account. Follow these instructions if you do not already have a Google or Gmail account.
